Class Registration Form

Please fill out this form to indicate your interest in registering for our school. You will be contacted using the method selected at the bottom of the form.

Classes are first come, first served and will close when class student limit is reached.

NOTE: Your successful completion of this registration form will hold your place in line for the classes you have selected. You will be contacted by Shelly to confirm your spot and to arrange payment. A registration fee of $50 per student will apply. Thanks!
